Adding eForm(s) to a Website

Below is a summary of the steps required to setup an eRequest link (i.e. an eForm that can be completed in a web browser). For more detailed instructions, please refer to the eRequest setup guide, located on our main website.

STEP 1: Register your clinic on the Ocean eReferral Directory.

STEP 2: Set up an eRequest link in the Admin tab of the Ocean portal.

STEP 3: Update your website page to link to your Ocean eRequest URL.

STEP 4: Sign into the Ocean portal to view your eRequests inbox.
